Samenvatting

De inzichten in de opbouw van de behandeling bij reumatoïde artritis zijn de laatste jaren veranderd. Vroeger begon reumabehandeling met gedoseerde rust, met passieve en actieve fysiotherapie, met voorlichting en met NSAID's. Met tweedelijnsmiddelen, disease modifying of slowacting anti-rheumatic drugs (DMARD of SA ARD) begon men pas na enkele jaren, als gewrichtsschade meestal al röntgenologisch aantoonbaar was. Achterliggende gedachte was dat reuma een relatief goedaardige ziekte is en dat de DMARD vrij toxisch zijn.
