Samenvatting

Multipele pathologie en dus de (kans op) polyfarmacie nemen toe. De inname van meerdere medicamenten tegelijk introduceert dan het risico van ongewenste interactieinteracties. Zeker bij middelen met een smalle therapeutische breedte kan dit ernstige gevolgen voor de patiënt hebben. Dit stelt dus hoge eisen aan een farmaceutische patiëntenzorg. Functioneert deze farmaceutische zorg of medicatiebewakingmedicatiebewaking ook zo?
