□ Zusammenfassung
Hintergrund: Fluorchinolone werden bei der kalkulierten Antibiotikatherapie von ambulant erworbenen Atemwegsinfektionen in den Empfehlungen der American Thoracic Society als gleichwertig zu den Makroliden und dem Doxycyclin gesehen, während die Deutsche Gesellschaft für Pneumologie Fluorchinolonen für diese Indikation keine wesentliche Bedeutung beimißt.
□ Material und Methoden
Um zu erfahren, ob diese unterschiedlichen Therapieempfehlungen durch die aktuelle Resistenzlage noch gestützt werden, haben wir im Rahmen einer multizentrischen Studie klinisch relevante Isolate von Streptococcus pneumoniae, Haemophilus influenzae und Moraxella catarrhalis hinsichtlich ihrer Antibiotikaempfindlichkeit unter besonderer Berücksichtigung von Sparfloxacin und Ciprofloxacin entsprechend den DIN-Empfehlungen untersucht.
□ Ergebnisse
Im Gegensatz zu vielen anderen europäischen Staaten und den USA ist die Prävalenz von Antibiotikaresistenzen in Deutschland noch niedrig. Betalactamantibiotika zeigten bei Haemophilus influenzae und Streptococcus pneumoniae in über 95% Wirksamkeit, jedoch waren 10% der Streptococcus-pneumoniae-Isolate gegenüber Tetracyclin und 7% gegenüber Erythromycin resistent. Von den beiden getesteten Chinolonantibiotika war das neuere Fluorchinolon Sparfloxacin stets wirksamer als Ciprofloxacin; Resistenzen gegenüber den Chinolonen waren nicht nachweisbar.
□ Schlußfolgerungen
Diese Empfindlichkeitsdaten zusammen mit der bekannten Prävalenz anderer pulmopathogener Infektionserreger, wie zum Beispiel Chlamydia pneumoniae und Mycoplasma pneumoniae, die stets sensibel für Erythromycin und Tetracyclin sind, unterstützen die Empfehlungen der Deutschen Gesellschaft für Pneumologie. Neuere Fluorchinolone mit verbesserter Pneumokokkenaktivität sind jedoch bei Patienten mit länger dauernden bis häufigeren Infektexazerbationen oder bei Patienten mit bedeutsamen Grunderkrankungen oder Risikofaktoren eine wichtige Alternative.
Abstract
□ Background
The American Thoracic Society regards fluoroquinolones together with macrolides and doxycyclin as first choice antibiotics in the empirical treatment of community acquired pneumoniae in non-hospitalized patients, while the Deutsche Gesellschaft für Pneumologie only recommends macrolides and doxycyclin for these patients.
□ Material and Methods
In order to find out if the German recommendations still adequately reflect the local resistance situation, we analyzed antibiotic resistance of clinically relevant isolates of Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis in a multicenter study with special reference to sparfloxacin and ciprofloxacin according to DIN recommendations.
□ Results
In contrast to other European countries and the USA the prevalence of antibiotic resistance in Germany is low. Betalactam antibiotics were effective against more than 95% of strains of Haemophilus influenzae and Streptococcus pneumoniae. However, 10% of the strains of Streptococcus pneumoniae were resistant to tetracyclin and 7% resistant to erythromycin. Resistances against ciprofloxacin and sparfloxacin were not detectable. Of both quinolone antibiotics, sparfloxacin was always more active than ciprofloxacin.
□ Conclusions
These susceptibility data and the known prevalence of other respiratory tract pathogens such as Chlamydia pneumoniae and Mycoplasma pneumoniae which are sensitive to erythromycin and tetracyclin therefore support the recommendations of the Deutsche Gesellschaft für Pneumologie. However, newer fluoroquinolones with increased activity against pneumococci may be a helpful alternative for patients with persistent and recurrent exacerbations of respiratory infections and patients with relevant underlying diseases or relevant risk factors.
