Zusammenfassung
□ Problem
Es gibt epidemiologische, klinische und experimentelle Hinweise, daß Jod eine Autoimmunthyreoiditis induzieren oder deren Manifestation begünstigen kann. Vor diesem Hintergrund ist von Interesse, ob sich beim Ausgleich des alimentären Jodmangels oder bei der Jodtherapie der Jodmangelstruma Schilddrüsenantikörper entwickeln und welche praktische Bedeutung diesem Phänomen beigemessen werden muß.
□ Patienten und Methoden
In einer prospektiven Studie untersuchten wir 209 Patienten mit euthyreoter Jodmangelstruma und 53 gesunde Probanden. Die Behandlung erfolgte mit 200 μg Jod/Tag (n=119), 500 μg Jod/Tag (n=27), 1,5 mg Jod/Woche (n=41), 150 μg Jod plus 75 bis 100 μg T4/Tag (n=26), 100 μg Jod plus 100 μg T4/Tag (n=25) und 100 μg T4/Tag (n=24). Der Beobachtungszeitraum betrug ein Jahr. Die Kontrollen erfolgten alle drei Monate, und sie umfaßten: klinische Untersuchung, Sonographie, TSH, T3, FT4, TPO-Antikörper, Thyreoglobulinantikörper und Jodurie.
□ Ergebnisse
Nach zwölf Monaten hatten sich bei 7,5% der mit Jod behandelten Fälle Antikörper entwickelt. Meist handelte es sich um niedrige Titer. Bei Gesunden fanden sich in 3,8%, bei Strumapatienten in 9,0% und bei nodösen Strumen in 11,1% erhöhte Antikörperbefunde. 500 μg Jod lösten mit 14,8% am häufigsten Antikörperreaktionen aus. Nach 200 μg Jod/Tag waren 5,0% antikörperpositiv. T4 scheint Antikörperreaktionen zu dämpfen. Pathologische Antikörperwerte fanden sich bei den Patienten, bei denen eine Jod/T4-Therapie oder T4-Monotheapie durchgeführt wurde, nicht. Von den 22 Patienten mit primär pathologischen Antikörperbefunden zeigten nur vier einen Anstieg (18,2%). Das betraf allein drei von fünf Patienten in der Gruppe mit 500 μg Jod/Tag. Bei fünf Patienten normalisierten sich die primär hohen Werte (22,7%). Hypothyreote Funktionsstörungen konnten in keinem Fall beobachtet werden. Die Sonographie ließ keine Veränderungen erkennen, und die Volumenreduktion war bei Patienten mit Antikörpern nicht beeinträchtigt. Die renale Jodausscheidung lag im Untersuchungszeitraum im Median bei 5,2 bis 7,2 μg/dl.
□ Schlußfolgerungen
Insgesamt muß den möglichen Antikörperreaktionen kein besonderer Krankheitswert beigemessen werden. Im Einzelfall bedürfen die Erscheinungen jedoch der Kontrolle. Niedrige Joddosen (bis 200 μg/Tag) sollten bevorzugt werden. Die kombinierte Jod/T4-Therapie scheint im Hinblick auf mögliche immunologische thyreoidale Reaktionen von Vorteil zu sein.

□ Problem
There is epidemiological and clinical evidence that iodine may induce or promote the manifestation of autoimmune thyroiditis. For this reason it is important to know if substitution of alimentary iodine deficiency or iodine treatment of endemic goitre can cause formation of thyroid antibodies. On the other hand the practical importance of this phenomenon should be evaluated.
□ Patients and methods
During a prospective study we examined 209 patients with endemic non-toxic goitre and 53 healthy people. For treatment were used 200 μg iodine/d (n=119), 500 μg iodine/d (n=27), 1,5 mg iodine/week (n=41), 150 μg iodine/d plus 75 to 100 μg T4/d (n=26), 100 μg iodine plus 100 μg T4/d (n=24). The observation took 1 year with a 3-month intervall for check ups including clinical examination, ultrasound, TSH, T3, fT4, TPO- and thyreoglobuline antibodies and urinary iodine.
□ Results
After 12 months 7.5% of iodine treated persons had produced antibodies, most of them at low levels. In healthy people we found increased antibody-levels in 3.8%, in patients with goitre in 9.0%, in patients with nodular goitres in 11.1%. 500 μg iodine caused the most antibody reaction in 14.8%. People treated with 200 μg iodine/d showed positive antibody levels in 5%. T4 seems to reduce antibody-reactions. Pathological antibody-levels were not found in patients with combined iodine/T4- and single-T4 therapy. Among the 22 primary pathological antibody levels only 4 increased further (18.2%). Three of them belonged to the group of 5 persons treated with 500 μg iodine/d. Primary high antibody values were normalized in 5 patients (22.7%). Hypothyroid disturbances were not found. Ultrasound did not show any alterations, and the reduction of thyroid volumes in antibody-positive patients was not affected. Median urinary iodine excretion during the observation-intervall was 5.2 to 7.2 μg/dl.
□ Conclusions
Possible antibody reactions have no clinical importance at all. Individual cases must be observed. Low iodine doses should be prefered. Combined iodine/T4 treatment seems to have an advantage regarding immunological thyroidal reactions.
