Intestinal neuronal dysplasia — a disease with surgical relevance

Zusammenfassung

Die intestinale neuronale Dysplasie (IND) ist eine Entwicklungsstörung des Plexus submucosus des Kolons, die sowohl bei Kindern als auch bei Erwachsenen gefunden werden kann. Die Diagnose kann mittels einer enzymhistochemischen Lactatdehydrogenasereaktion an nativen Biopsien aus dem Rektosigmoid zuverlässig gestellt werden.

In einer klinischen Studie ließ sich bei Patienten mit primär chronischer Obstipation (n=56) eine neuronale Kolondysplasie (47/56) nachweisen, während beim Kontrollkollektiv (0/15) eine normale Innervation der Darmwand vorlag (p<0,001).

Die primär chronische Obstipation bei intestinaler neuronaler Dysplasie ist für konservative Therapiemaßnahmen meist nicht zugänglich, so daß als therapeutische Konsequenz eine chirurgische Intervention notwendig ist.

Abstract

Intestinal neuronal dysplasia is an abnormality of the development of the submucous plexus of the colon which can be found in children and in adults. The diagnosis can reliably be made by enzymhistochemical examination with lactic dehydrogenase in biopsies from the rectosigmoid.

In a clinical study biopsies of patients with primary chronic constipation showed the characteristics of intestinal neuronal dysplasia (47/56). Biopsies of the control group had a normal gut innervation (0/15; p<0,001). Primary chronic constipation with intestinal neuronal dysplasia cannot be treated successfully by conservative measurements. Therefore, a surgical intervention is often unavoidable.
