Zusammenfassung
Die Therapie von Herzrhythmusstörungen hat in den letzten Jahren eine enorme Wandlung durchgemacht. Dies ist nicht zuletzt Folge großangelegter kontrollierter Studien zur Wirksamkeit antiarrhythmischer Therapieverfahren. In diesen Studien wurde und wird überprüft, ob eine antiarrhythmische Maßnahme die Häufigkeit des plötzlichen Herztodes vermindern und die Lebenserwartung der Patienten verbessern kann. Durchführung und Publikation solcher Studien sind noch nicht abgeschlossen. Es ist daher zu erwarten, daß sich die antiarrhythmische Therapie in den nächsten Jahren als Folge neuer Studienergebnisse weiter verändern wird.
Die Thematik dieser Studien läßt sich folgendermaßen klassifizieren:

- Prävention des plötzlichen Herztodes nach akutem Myokardinfarkt durch Klasse-III-Antiarrhythmika — Amiodaron, d-Sotalol, Dofetilide,
-
- Prävention des plötzlichen Herztodes bei linksventrikulärer Dysfunktion/Herzinsuffizienz durch Klasse-III-Antiarrhythmika — Amiodaron, Dofetilide,
-
- Prävention des plötzlichen Herztodes bei Risikopatienten nach akutem Myokardinfarkt — Vergleich zwischen medikamentöser Therapie und implantierbarem Cardioverter/Defibrillator (ICD),
-
- Prävention des plötzlichen Herztodes bei bestimmten Risikopatienten; Vergleich der medikamentösen Therapie und des implantierbaren Cardioverter/Defibrillators, zum Beispiel bei Patienten mit idiopathischer dilatativer Kardiomyopathie, anhaltenden Kammertachykardien und auf der Warteliste zur Herztransplantation,

- Sekundärprävention des plötzlichen Herztodes nach überlebtem tachykarden Herzstillstand — Vergleich medikamentöser Maßnahmen mit der Implantation des implantierbaren Cardioverter/Defibrillators.
Es ist entscheidend, daß in diesen Studien nicht nur der Einfluß der Therapiemaßnahme auf den plötzlichen Herztod, sondern auch auf die Gesamtsterblichkeit geprüft wird.
Summary
When dealing with preliminary or brandnew results of large randomised trials, the requirement should be the assessment of the data by experts in the arrhythmia field. This holds especially true for studies addressing the complex problem of the prevention of sudden cardiac death. During recent years a number of controlled randomised trials have been published showing no or a harmful effect of antiarrhythmic drugs on the prognoses of patients at risk of sudden cardiac death. The results of these studies have clearly changed the scenario of preventive antiarrhythmic drug treatments. Some newer studies — published only in a preliminary form — seem to indicate that some Class III antiarrhythmics can have a positive effect. Studies are under way to compare the preventive effect of this antiarrhythmic drug therapy against sudden cardiac death with that of the implantable cardioverter/defibrillator.
The improvement of prognosis remains the ultimate goal of antiarrhythmic therapy.
