Pharmacological treatment of atrial arrythmias

Zusammenfassung

Die medikamentöse Therapie tachykarder Vorhofarrhythmien hat klar definierte Indikationen der Akutbehandlung der AV-Knoten-Reentry-Tachykardie, der atrioventrikulären Reentry-Tachykardie und der akuten und chronischen Therapie von Vorhofflimmern in solchen Fällen, in denen als Behandlungsziel eine Verlangsamung schneller AV-Überleitungen der Erregung verfolgt werden. Fakultative Therapieindikationen sind die chronische Behandlung der AV-Knoten-Reentry-Tachykardie, die Rhythmisierungsbehandlung von Vorhofflimmern und die medikamentöse Rezidivprophylaxe bei Vorhofflimmern.

In einer mittelfristigen Perspektive ist keine Ausweitung der medikamentösen Therapieindikationen zu erkennen; es ist vielmehr zu erwarten, daß nichtpharmakologische Behandlungsverfahren an Bedeutung zunehmen werden.

Summary

There are defined indications for the acute pharmacological management of AV-nodal reentry tachycardias, AV reentry tachycardias and for the acute and chronic ventricular rate control in atrial fibrillation. Possible indications arise for the chronic pharmacologic therapy of AV reentry tachycardias, pharmacological cardioversion and prophylaxis of atrial fibrillation.

In future there will be a trend towards nonpharmacological management of atrial arrhythmias.
