Zusammenfassung
Zweck der Studie war, die Wertigkeit der dreidimensionalen Echokardiographie in der Beurteilung angeborener Mißbildungen der Mitralklappe zu prüfen. Dazu wurden 11 Patienten im mittleren Alter von 8,3 (3–17,5) Jahren untersucht. Zu den untersuchten angeborenen Mitralklappenfehlern gehörten Mitralklappenprolaps, Mitralstenose durch abnorm verkürzte Papillarmuskel und „parachute” Mitralklappe. Die zur dreidimensionalen Rekonstruktion notwendigen multiplen sequentiellen tomographischen Querschnitte wurden alle von transthorakal her aufgenommen. Bei allen Patienten gelang eine dreidimensionale Rekonstruktion der aufgenommen Datensätze. Es konnten sämtliche von der konventionellen Schnittbildechokardiographie her bekannten Schnittebenen und neue Schnittebenen dargestellt werden. Als besonders hilfreich in der Beurteilung der Mitralklappe erwies sich dabei die Aufsicht auf die Klappe vom linken Vorhof aus sowie Abbiildungsebenen, welche durch Drehung des Vierkammerblickes um die horizontale Achse erhalten werden können, welche eine bessere Beurteilung der Mitralklappensegel und der Papillarmuskel ermöglichen als die konventionelle Echokardiographie. Wir schlußfolgern, daß die dynamische dreidimensionale Echokardiographie zur Beurteilung der Mitralklappenmorphologie neue Informationen liefert.
Summary
The purpose of this study is to examine the value of dynamic three-dimensional echocardiography in the assessment of congenital mitral valve lesions. We examined 11 patients with a mean age of 8.3 (3–17.5) years. Among the congenital mitral valve anomalies we encountered were mitral valve prolapse with incompetent mitral valve, and mitral valve stenosis by abnormally shortened papillary muscles as well as „parachute” mitral valve. The multiple sequential cross-sections required for three-dimensional reconstruction were acquired with transthoracic scanning using either rotational or fan-like scanning. Three-dimensional reconstruction was possible in all patients. All echocardiographic views of the mitral valve available by conventional transthoracic echocardiography could be reconstructed as well as new views. Specifically, the „en-face” view of the mitral valve from the left atrium and views of the mitral valve from the atrium and apex obtained by rotating the dataset around the horizontal axis in a conventional four-chamber view
