Zusammenfenssung
□ Anamnese
Bei einem 1934 geborenen Mann wird im Alter von 32 Jahren eine „benigne” monoklonale Gammopathie vom Leichtkettentyp Kappa mit Bence-Jones-Proteinurie diagnostiziert Zusätzlich liegt ein Noonan-Syndrom vor.
□ Verlauf
Nach über 24jährigem unverändertem Verlauf entwickelt sich allmählich eine bis Oktober 1996 noch nicht behandlungsbedürftige chronisch lymphatische Leukämie (B-CLL). Die neoplastischen B-Zellen exprimieren monoklonales Leichtketten-Lambda auf der Zellmembran und im Zytoplasma, ohne daß dies bei der Immunfixation im Serum nachweisbar ist. Davon unabhängig exprimiert die bekannte monoklonale Gammopathie IgG-Kappa, ohne daß Plasmazellen im Knochenmark vermehrt sind.
□ Schlußfolgerung
Es ergeben sich Hinweise darauf, daß das angeborene Noonan-Syndrom von B-Zell-Störungen begleitet sein kann.
Abstract
□ History
At the age of 32 a “benign” monoclonal gammopathy of lightchain kappa with Bence Jones protein is diagnosed in a man born 1934. In addition a Noonan-syndrome is found.
□ Course
Twenty-four years later he gradually develops a chronic lymphatic leukaemia (B-CLL) which up to now does not need treatment (October 1996). The neoplastic B-cells exprime monoclonal lightchain lambda on the cellmembrane and in the cytoplasma undetectable by immunefixation in the serum. Irrespective of that the known monoclonal gammopathy exprimes IgG-kappa without an increase in the number of plasmacells in the bonemarrow.
□ Conclusion
There are hints that the congenital Noonan-syndrome can be associated with B-cell disorders.
