Zusammenfassung
Bei 49 Patienten, die mit biventrikulären Kreislaufunterstützungssystemen (BVAD) unterstützt wurden, wurden regelmäßige Bestimmungen der Konzentration der Elastase, des Komplementfaktors C3a und von Endotoxinen durchgeführt. Die Patienten wurden hinsichtlich des Erreichens bzw. Nichterreichens des Therapieziels (erfolgreiche Herztransplantation) zwei Gruppen zugeteilt. Gruppe I transplantiert, und Gruppe II am Unterstützungssystem verstorben. Die gemessenen, Konzentrationen der genannten Parameter wurden mit klinischen und laborchemischen Daten des Verlaufes der Patienten in den beiden Gruppen verglichen.
Es zeigte sich, daß die am BVAD verstorbenen Patienten deutlich höhere Konzentrationen an Elastase sowie Komplementfaktor C3a aufwiesen und diese mit der Verschlechterung von Organfunktionen und dem Auftreten einer Infektion korrelierten. Die Endotoxinkonzentration der beiden Gruppen unterschieden sich ebenfalls signifikant. Die Konzentration von Elastase sowie Komplementfaktor C3a erlauben es bei Patienten an mechanischen Unterstützungssystemen, die Diagnose Infektion bzw. Sepsis zu untermauern. Ferner können anhand der Höhe dieser Parameter der Infektionsverlauf sowie die Prognose abgeschätzt werden. Diese Parameter können eine relevante Hilfestellung zur Bestimmung des idealen Transplantationszeitpunktes bei Patienten an mechanischen Unterstützungssystemen geben.
Summary
Elastase, complement factor C3a and endotoxin were measured in 47 patients supported with biventricular assist devices (BVAD). Patients were divided into two groups according to outcome: group I patients reached heart transplantation, group II patients died while on mechanical assist device support. The course of the parameters mentioned above were correlated with the patients clinical course. Patients dying while on mechanical support had elevated concentrations of elastase and complement factor C3a. This correlated to deteriorating organ function and the advent of infectious complications. Endotoxin concentrations were significantly different between the two groups as well.
Increased levels of elastase and complement C3a can add to the diagnosis of sepsis in patients on mechanical support systems. The course and prognosis of infectious complications can be estimated by the level of these parameters. The transplantability of patients supported with mechanical assist devices can be assessed by taking these parameters into account.
