Zusammenfassung
Ziel
Bei Patienten mit euthyreoter Struma wurde die Wirksamkeit von 400 μg Jodid und der Kombination von 100 μg Levothyroxin mit 100 μg Jodid versus dem bisherigen Therapiestandard, der individuell dosierten Levothyroxin-Gabe, verglichen.
Patienten und Methoden
Insgesamt 78 Patienten mit euthyreoter diffuser Struma (≥25 ml) wurden prospektiv eingeschlossen, randomisiert und für sechs Monate behandelt. Der Verlauf der Schilddrüsengröße wurde mittels sonographischer Volumetrie verfolgt.
Ergebnisse
Die Daten von 69 Patienten waren auswertbar (57 Frauen, zwölf Männer, Alter 31±1 Jahre, Schilddrüsenvolumen 31,5±1,4 ml, 23 je Therapiegruppe). In der mit individuell dosiertem Levothyroxin behandelten Gruppe nahm das Schilddrüsenvolumen um insgesamt 39% ab (95%-Konfidenzintervall [KI] −31% bis −41%). Die Volumenreduktionen, die im gleichen Zeitraum mit Mono-Jodid oder mit der Levothyroxin/Jodid-Kombination erreicht wurden, waren hiervon jedoch nicht signifikant unterschiedlich (p=0,35, Varianzanalyse, Mono-Jodid −34%, 95%-KI −29% bis −43%, 100 μg Levothyroxin/100 μg Jodid −39%, 95%-KI −32% bis −45%).
Schlußfolgerungen
In der Therapie der euthyreoten diffusen Struma kann somit der alleinigen Jodidgabe als auch der Kombination von Levothyroxin mit Jodid prinzipiell der gleiche Stellenwert wie bisher der individuell dosierten Levothyroxin-Behandlung beigemessen werden. Aus Sicht der Autoren erscheint ihre bevorzugte Behandlung mit Mono-Jodid jedoch am sinnvollsten.

Aim
In patients with euthyroid goitre, the efficacy of treatment with 400 μg iodine and 100 μg levothyroxine combined with 100 μg iodine was compared to that of the previous standard of therapy, individually dosed levothyroxine.
Patients and Methods
A total of 78 patients presenting with euthyroid diffuse goitre (≥25 ml) were prospectively enrolled, randomised and treated for 6 months. The course of thyroid volume was followed using thyroid volumetry.
Results
Data of 69 patients were included in the final evaluation (57 women, 12 men, age 31±1 years, thyroid volume 31,5±1,4 ml, 23 per treatment group). In the patients treated with individually dosed levothyroxine, the thyroid volume decreased by about 39% (95%-confidence limit [CL] −31% to −41%). However, the volume reductions achieved in the patients treated with 400 μg iodine or 100 μg levothyroxine/100 μg iodine were not significantly different (p=0,35, variance analysis, mono-iodine −34%, 95%-CL −29% to −43%, 100 μg levothyroxine/100 μg iodine −39%, 95%-CL −32% to −45%).
Conclusion
In patients with euthyroid diffuse goitre, treatment with mono-iodine or combination of levothyroxine with iodine should have principally the same status as the previous standard of therapy, individually dosed levothyroxine. In the view of the authors, its preferential treatment with mono-iodine appears most reasonable.
