Zusammenfassung
Im ersten Halbjahr 1997 wurden 414 Stuhlproben von 58 EHEC-infizierten Personen mit verschiedenen diagnostischen Methoden untersucht. Zum EHEC-Toxinnachweis wurde ein ELISA und der Verozelltest eingesetzt, Toxingene wurden mit der PCR nachgewiesen. Mit der IMS wurden die Stuhlproben aufE. coli der Serogruppe O157 untersucht.
Bei insgesamt 272 Proben wurden phänotypisch und/oder genotypisch Shigatoxin nachgewiesen. Die Nachweisverfahren für Toxin und Toxingene ergaben in 60 % der Fälle ein übereinstimmendes positives Ergebnis, während bei den restlichen Proben meist der ELISA und/oder der Verozelltest negativ ausfielen. Bei 61 Proben war nur mit der PCR der Nachweis einer EHEC-Infektion möglich. Durch Untersuchung von mehrerenE. coli-Einzelkolonien konnten bei 51 Personen EHEC-Stämme isoliert und einer Serotypisierung zugeführt werden. Die Stämme gehörten zu 22 verschiedenen O-Gruppen. EHEC der Serogruppe O157 wurden dabei dreimal isoliert. Von den O157-positiven Personen wurden nur sieben Stuhlproben untersucht, so daß ein aussagekräftiger Vergleich der IMS mit anderen Nachweisverfahren nicht möglich war.
Summary
In the first six months of 1997 414 stool specimens from 58 persons infected with enterohaemorrhagic Escherichia coli (EHEC) were examined by different diagnostic methods. EHEC-toxin was detected by ELISA and verocelltest, toxingenes by PCR andE. coli O157 by IMS.
Shigatoxin and/or toxingenes were found in 272 stool specimens. Consistent positive results by all diagnostic methods that detect toxin or toxingenes were obtained in 60 %, in the rest of the specimens ELISA and/or verocelltest were negative. In 61 specimens the detection of an EHEC-infection was possible only by PCR. By analyzing several singleE. coli-colonies EHEC-strains were isolated and serotyped from 51 persons. The strains belonged to 22 different O-groups. EHEC of serogroup O157 was isolated three times. From O157-positive persons only seven stool specimens were analysed and therefore a reliable comparison of IMS with the other detection methods was not possible.
