Zusammenfassung

Trotz intensiver Bemühungen von Fachgesellschaften, der WHO/IDF Europa und vielen Einzelinitiativen von diabetologischen Experten ist es bisher nicht gelungen das einfache, effektive und relativ billige Screening-Programm auf diabetische Komplikationen in breitem Stil in der Bundesrepublik Deutschland durchzusetzen, obgleich die Betroffenen und unser Gesundheitssystem spätestens seit der St. Vincent Initiative 1989 darauf Recht hätten, da bei frühzeitiger Entdeckung beginnender diabetischer Komplikationen heute wirksame therapeutische Möglichkeiten zur Verfügung stehen. Das Screening besteht aus regelmäßiger Fußinspektion, einfacher vaskulärer und neurologischer, sowie gründlicher ophthalmologischer Untersuchung, Erarbeitung des vaskulären Riskoprofils (Rauchen, glykiertes Hämoglobin, Blutdruck, Köpergewicht, Triglyceride, Gesamt- und HDL-Cholesterin), EKG, evtl. Doppler-Untersuchung und Screening auf Mikroalbuminurie. Ein entsprechendes Screening-Programm zum Nachweis einer Mikroalbuminurie (Prosit Projekt) wird vorgestellt.

Summary

Despite of intensive efforts of diabetes associations, the WHO/IDF Europe and many initiatives of diabetes specialists the simple, effective and rather cheap screening programme for diabetic complications is still not functioning, although the people with diabetes and our health care system the latest since the St. Vincent Initiative in 1989 have the right for this screening, because there are effective therapeutic measures. The screening consists of regular foot inspection, simple vascular and neurological as well as careful ophthalmological examinations, screening of the vascular risk profile (smoking, glycated hemoglobin, blood pressure, body weight, triglycerides, total and HDL-cholesterol), ECG (Doppler examination), and screening for microalbuminuria. The screening programme for microalbuminuria (Prosit Projekt) will be presented.
