L’approche morphologique des spermatozoïdes à un fort grossissement appelée MSOME (Motile Sperm Organelle Morphology Examination) constitue un progrès intéressant. Elle permet de détecter sur cellules vivantes, avec un grossissement allant jusqu’à plus de X10 000, des anomalies non visualisables par les autres techniques. Cette technique a été d’emblée appliquée à la sélection du spermatozoïde injecté en ICSI, prenant alors le nom d’IMSI (Intracytoplasmic Morphologically Selected sperm Injection), avec un bénéfice très intéressant concernant les taux d’implantation, de grossesse, d’accouchement ainsi qu’une diminution des taux de fausses couches spontanées.
En dehors de la nécessité d’un système optique particulier, les contraintes techniques sont importantes et l’observation, en MSOME et en IMSI, sont plus longues et difficiles dans les cas de cryptozoospermie, d’oligozoospermie et/ou de tératozoospermie importantes.
Comparativement au spermocytogramme conventionnel (sur cellules fixées et colorées), le MSOME offre une excellente lisibilité des anomalies céphaliques, notamment de vacuoles localisées dans la région céphalique et qui semblent avoir un effet délétère sur la fécondation et le développement embryonnaire. Ces observations conduisent à utiliser une grille de lecture détaillée des anomalies, et ce diagnostic morphologique sur spermatozoïdes vivants montre qu’un pourcentage notable des spermatozoïdes pré-sélectionnés pour une ICSI conventionnelle sont en fait anormaux en MSOME.
Au delà de ces résultats incontestables au plan thérapeutique, cette nouvelle approche ouvre des perspectives très intéressantes sur les relations à établir entre le phénotype du spermatozoïde injecté et son pouvoir fécondant et de développement embryonnaire. Au plan diagnostique, le MSOME ouvre la perspective de pouvoir étudier avec nos outils habituels, des populations morphologiquement homogènes de spermatozoïdes normaux ou présentant une anomalie ciblée. De telles investigations et l’analyse morphologique fine de chaque spermatozoïde injecté associée au suivi de chaque ovocyte, puis au suivi embryon par embryon, permettraient de progresser dans la relation structure-fonction du spermatozoïde, l’identification des indications pertinentes de l’IMSI et le choix du spermatozoïde à injecter.
Abstract
Motile Sperm Organelle Morphology Examination (MSOME) constitutes a real improvement in ART management and outcome, as it allows detection of specific sperm anomalies on living cells, which cannot be detected by routine analysis. MSOME applied to the selection of sperm injected into the oocyte is called IMSI (Intracytoplasmic Morphologically Selected sperm Injection) and is associated with a considerable improvement of implantation, clinical pregnancy and delivery rates.
A high-power microscope (X 2,000 to X 10,000) and video enhancement system are necessary and technical limitations are related to cryptozoospermia and/or severe teratozoospermia.
Compared to routine sperm morphology assessment, MSOME allows the detection of subtle cephalic anomalies, such as vacuoles. These vacuoles seem to have a deleterious effect on fertilization and embryo developmentin vitro. These observations have led to a detailed classification of anomalies and this morphological diagnosis on living sperm demonstrates that most sperm selected for conventional ICSI at X 400 are actually abnormal on MSOME at X 6,000 or more.
In addition to the very good results obtained in IMSI, this new approach opens up interesting prospects concerning the relationship between the phenotype of the injected sperm and its fertilization capacity and embryo development. In terms of diagnosis, MSOME could be used to select and study homogeneous groups of normal sperm or homogeneous groups of sperm exhibiting the same well defined anomaly. Such studies, associated with fine analysis of injected sperm and follow-up of each oocyte and each embryo, should provide more information about the relationship between sperm structure and function and should help to define the relevant indications for IMSI and the choice of spermatozoa to be injected.
