Résumé
Nous avons présenté un cas d’œdème aigu pulmonaire fulminant non-cardiogénique survenant en fin d’anesthésie. Ce diagnostic précis a pu être posé par la mesure de la pression capillaire bloquée et du débit cardiaque ainsi que par l’analyse du liquide d’œdème alvéolaire. Les valeurs hémodynamiques et de l’échange gazeux mesurées sous différents modes ventilatoires confirment l’efficacité du traitement par la ventilation à pression positive continue (CPPV). Dans la genèse de l’œdème aigu pulmonaire décrit, le rôle de l’hypoxie et de l’obstruction aiguë des voies aériennes hautes présentées par le patient est discuté à la lumière des théories proposées actuellement.
Summary
We have presented a case of acute fulminating pulmonary œdema, not cardiogenic in origin, occurring at the end of anaesthesia. This precise diagnosis was made with the help of measurement of the capillary wedge pressure and cardiac output as well as analysis of the alevolar oedema fluid. The haemodynamic values and the gas exchange, as measured under various ventilatory patterns, confirms the usefulness of continuous positive pressure ventilation (CPPV).
In the genesis of acute pulmonary oedema as described, the role of hypoxia and acute obstruction of the upper airways as presented by the patient is discussed in the light of the present proposed theories.
