Les troubles fonctionnels digestifs (TFD) sont très fréquents en Afrique, comme dans le reste du monde. Il faut regrouper dans ce contexte nosologique à la fois les dyspepsies, le syndrome de l’intestin irritable (SII), les troubles fonctionnels intestinaux (TFI), les colopathies fonctionnelles.
En Afrique, les tableaux symptomatiques sont plus riches, avec des descriptions tellement imagées que le praticien peut être désorienté.
En outre, les croyances traditionnelles et ancestrales, les sorciers, les guérisseurs, les fétichistes, les pharmacies parallèles et l’automédication viennent compliquer considérablement la prise en charge de ces malades. Il en est de même pour les problèmes de bas niveau socioéconomique, de malnutrition, d’hygiène et de mauvaise maîtrise de l’eau.
Les principaux motifs de consultation sont la constipation, les divers problèmes de gaz et les douleurs abdominales. Le risque de multiplier des examens inutiles est grand devant ces malades avides d’explorations sophistiquées mais aussi devant la peur de méconnaître une affection organique.
La très difficile prise en charge thérapeutique, souvent prolongée, fait appel à de la psychothérapie, à des mesures hygiéno-diététiques (avec des difficultés à bien codifier des régimes avec les aliments africains), et à des médicaments parmi lesquels les prokinétiques comme la trimébutine sont très utiles.
Summary
Functional bowel disease is as common in Africa as in the rest of the world. Other names used to describe this disorder include functional dyspepsia, irritable bowel syndrome, spastic colon and functional colitis.
In Africa, the clinical presentation is varied and could be so dramatic as to confuse some doctors. More over the management of these patients is complicated by a number of factors: traditional beliefs and practices, roadside pharmacies and automedications. Other problems related to low socio economic level, malnutrition, poor hygiene and poor treatment of water make things worse.
The major presenting complaints are constipation, abdominal pains and gas. There is a risk of requesting unnecessary tests in this patients who are eager to have sophisticated investigations, but also afraid of underminding an organic illness.
The management is difficult and often long. It includes from psychotherapy to dietary measures (with difficulty to define diets with African foodstuffs), and drugs among which prokinetics such as trimebutine are very useful.
