L’oesophago-gastro-duodenoscopie en pédiatrie

Résumé

Chez l’enfant, les difficultés de l’interrogatoire et de l’interprétation des clichés radiologiques justifient l’intérêt de l’endoscopie.

104 examens ont été réalisés chez 99 enfants d’âge inférieur à 16 ans.

Les appareils utilisés ont été les duodénoscopes axiaux ou latéraux, les axoscopes type GIF au-dessus de 10 ans; mais l’apparition des gastroscopes pédiatriques en fait l’appareil préférentiel. L’examen est réalisé sous prémédication dans 1 cas sur 2: un suppositoire de Nembutal paraît le plus souvent suffisant. Dans 4 cas, il n’a pas été possible de réaliser l’endoscopie.

Les résultats mettent en évidence:

• 1) la fréquence des examens normaux (44%) malgré le caractère parfois bruyant des symptômes hémorragiques ou vomissements;

• 2) dans 20% des cas, il a été découvert une maladie ulcéreuse de localisation duodénale dans 70% des cas;

• 3) la malposition la hernie hiatale avec ou non œsophagite représentent la deuxième étiologie (19% des cas).

Summary

In the child, endoscopy is interesting because it is difficult to question the patient and to read the X-rays. 104 examinations were performed on 99 children under 16. The instruments used were axo — and lateroduodenoscopes; the GIF type axoscopes were used for children above 10. But since their apparition the pediatric endoscopes are prefered.

The fiberendoscopy is performed with premedication in 50% of the cases: most of the time a «nembutal» suppository is sufficient.

In four cases it was impossible to realize the endoscopy.

The results show that:

• 1) The normal examinations are frequent (44%) in spite of the sometimes intense hemorrhage or vomiting.

• 2) In 20% of the cases an ulcer was found, of duodenal location in 70% of the cases.

• 3) Malposition, hiatal hernia with or without esophagitis represent the second etiology (19% of the cases).
