Fine needle aspiration biopsies under endoscopic ultrasonography (EUS)

Résumé

L’échoendoscopie (EE) permet de réaliser des ponctions extrêmement précises d’organes profonds, dans un but diagnostique. Les principales indications concernent les tumeurs primitives du pancréas, la recherche de métastases ganglionnaires et les métastases d’organes profonds.

Le matériel recueilli comprend des cytologies conventionnelles, des microbiopsies et éventuellement des cytologies monocouches qui permettent comme les microbiopsies de réaliser des examens complémentaires. Les ponctions du pancréas représentent le plus important recrutement, les indications des ponctions se justifient par la mise en place de protocoles thérapeutiques nécessitant un diagnostic précis avant tout acte thérapeutique. La sensibilité de la technique est meilleure pour les lésions solides que pour les lésions kystiques dont les prélèvements sont souvent peu informatifs.

Summary

Exact localization of the lesion to be biopsied is central to the successful performance of EUS guided biopsy. The potential indications for EUS-FNA include diagnosis of pancreatic lesions, lymph node staging, and biopsy of distant metastases. The material obtained, consists of smears, microbiopsy and monolayer cytology, microbiopsies and also monolayer cytology which is useful for complementary techniques. The main protocol interest pancreatic lesions. In the majority of cases, fine needle aspiration biopsy of the pancreas is used to confirm or exclude the presence of malignancy. New trial justified pretherapeutic accurate diagnosis. The sensitivity of the technique is better for solid mass than for cystic lesions.
