Résumé
L’écho endoscopie œsophagienne est sous-utilisée en milieu pneumologique. Ses indications principales se situent au niveau du bilan d’extension ganglionnaire des cancers broncho-pulmonaires et du diagnostic d’adénopathies médiastinales de cause inconnue. Elle explore particulièrement bien les régions sous-carénaire, para-œsophagienne et de la fenêtre aortico-pulmonaire et permet des ponctions à l’aiguille, guidées par les ultrasons avec une grande efficacité et une excellente sécurité. En ces sites, elle devrait être préférée à la ponction transthoracique sous scanner et devrait toujours précéder un abord chirurgical pour essayer de l’éviter.
Summary
Transesophageal endoscopic ultrasonography is underused in pneumologic practice. Best indications of this method appear to be N staging for lung cancers and diagnosis nature of mediastinal lymph nodes of unknown origin. It is particularly available for subcarina, paraesophageal and aortopulmonary window areas where fine-needle aspirations endosonography-guided may be obtained with high accuracy and excellent safety. In these locations, this method should be preferred to transthoracic aspiration CT-guided and should always precede surgical approach trying to avoid it.
