La possibilité de contamination croisée lors d’un acte d’endoscopie a toujours été prise en compte, mais les mesures préventives de désinfection du matériel et d’organisation des examens ont été renforcées en raison de l’irruption du danger représenté par la transmission possible du virus de l’immunodéficience humaine. A cette occasion, une réflexion plus générale sur le risque de transmission de divers microorganismes et l’efficacité des produits désinfectants a été conduite, entraînant une révision des procédures allant dans le sens de mesure plus sévères, dans le cadre de l’application du principe de précaution. Le suivi des taux de complications infectieuses montre une tendance à la baisse pour les incidents classiques. Les transmissions de virus restent heureusement des accidents extrêmement rares et leur faible résistance aux désinfectants en est une des raisons. Quant au risque de transmission du SIDA, il reste très théorique lorsque les bonnes pratiques sont suivies; celui de transmission de kystes de protozoaires est plus réaliste, mais très difficile à mettre en évidence.
Summary
The possibility of cross-contamination during an endoscopic exam has always been considered, but the preventive measures of disinfection of the material and organisation of the exams have been reinforced due to the new danger represented by a possible transmission of the Human Immunodeficiency Virus. In this context, a more general reflection on the risk of transmission of various micro-organisms and on the efficiency of disinfectants has been initiated, leading to a stricter revision of the procedures, following the application of the precautionary principle. The follow-up of the rate of infectious complications shows a trend of decreasing figures for classical incidents. Virus transmission, fortunately, remains an extremely seldom accident, likely resulting from their weak resistance to disinfectant. The risk of AIDS transmission remains very theoretical, when good practices are applied; the risk of transmission of protozoan cysts is more likely, but it is very difficult to document.
