Résumé
Les auteurs analysent les différentes causes de la sous-estimation de la douleur chez l'enfant qui peut expliquer une relative négligence dans l'attitude thérapeutique. De multiples facteurs rentrent en jeu et les auteurs insistent sur la valeur diagnostique de la douleur expliquant une attitude plutôt conservatrice, la relative ignorance de la physiologie douloureuse jusqu'à récemment, enfin la définition imprécise de l'enfance qui inclus la prématurité, la période néonatale, la petite enfance et l'adolescence. Deux concepts ignorés doivent être révisés: une prétendue inaptitude anatomique par insuffisance de myélinisation et/ou une densité basse des récepteurs de la douleur, une prétendue incapacité fonctionnelle due à un déficit d'un ou de plusieurs neurotransmetteurs. Les plus récents travaux biochimiques indiqueraient plutôt un déficit du contrôle central (voies descendantes, systèmes opioïdes). Ainsi, l'enfant souffre peut-être plus que l'adulte bien que sur un mode différent…L'intégration douloureuse est végétative d'abord, comportementale et cognitive plus tard d'où une spécificité de l'expression douloureuse dans la petite enfance en l'absence de communication verbale. Les auteurs concluent en évoquant le rôle de la douleur dans la maturation du système nerveux central.
Summary
The authors analyse the different causes of the underassessment of pain in child which can account for an relative negligence in therapeutic attitude. Multiple factors are involved and the authors insist upon the diagnostic value of pain explaining a rather respective attitude, the relative ignorance of pain physiology up to recently, the unprecise definition of «childhood» which includes prematurity, neonatal period, infancy, adolescence and so on… Two false concepts have also to be reviewed, an alleged anatomic inaptitude related to an insufficient myelinisation and/or alow density in pain receptors…, an alleged functionnal incapacity due to a lack in one or several neurotransmitters… The more recent biocheminal data would rather indicate a deficit in central pain control (descending pathways, opioïd systems). So the child suffers perhaps more than the adult does although on a different mode; the pain integration is vegetative at first, behavioral and cognitive lately, therefore a specificity of pain expression in infancy in the absence of verbal communication. Authors conclude recalling the role of pain in central nervous system maturation.
