Complications de la sphinctérotomie endoscopique

Conclusion

Les difficultés encore rencontrées pour la prévention des complications de la CPRE et de la sphinctérotomie endocopique ainsi que les progrès réalisés dans les autres techniques d'imagerie (Résonance magnétique, tomodensitométrie spiralée et surtout échoendoscopie) doivent nous faire réfléchir aux indications respectives de ces examens et à leur hiérarchisation en fonction des symptômes présentés par le patient.

A titre d'exemple, une sphinctérotomie pour micro-calculs de la voie biliaire principale se conçoit plus facilement en 1999, après la preuve de ce diagnostic par une écho-endoscopie.

Les complications de la sphinctérotomie endoscopique peuvent certainement être évitées ou leur gravité diminuée par la mise en application de quelques règles préventives simples.

Ceci suppose une pratique courante de la CPRE dans un centre organisé disposant d'un équipement radiologique performant et d'un personnel spécialisé.
