Conclusion
L'amélioration technique des minisondes permet maintenant de les utiliser de manière fiable et répétée dans les voies biliaires et pancréatiques. Elles sont utiles dans des indications où les autres techniques sont limitées: staging des tumeurs de Klatskin, staging complémentaire des ampullomes. Dans d'autres indications, elles ont une place potentielle qui doit être mieux précisée (diagnostic étiologique d'une sténose biliaire, TIPMP). Enfin dans certains cas (recherche tumeur endocrine, calcul biliaire …), des études complémentaires en comparaison avec l'EE standard sont certainement nécessaires avant de considérer qu'il s'agit de possibles indications.
L'avenir devrait voir leur place se préciser et leur utilisation déborder le champ des rares «échoendoscopistes cathétériseurs» pour s'étendre aux cathétériseurs. Il convient cependant de rappeler que la morbidité de l'acte sera dépendante du cathétérisme et toujours supérieure à celle d'une EE standard. Par ailleurs, leur profondeur de champ restera limitée. Ces deux limites ne pourront pas faire des minisondes une alternative à l'EE et elles resteront un examen limité à quelques indications précises.
