L'écho-endoscopie dans le diagnostic de la lithiase cholédocienne

Conclusion

L'échoendoscopie est, aujourd'hui, l'examen de référence pour la recherche de la lithiase de la voie biliaire principale grâce à ses performances diagnostiques (sensibilité, spécificité, valuer prédicitive positive et surtout valeur prédictive négative).

Elle permet, en outre, dans le même temps, une exploration de l'ensemble de la maladie lithiasique potentielle.

Sa morbidité en fait un examen semi-invasif pouvant être couplé, dans certaines conditions, à la réalisation d'une CPRE avec SE en cas de présence de lithiase de la voie biliaire principale, en particulier dans la prise en charge des pancréatites aiguës graves. Pour ces raisons, sa place demeure privilégiée malgré l'avènement des nouvelles techniques d'imagerie, en particulier la cholangio RMN.
