Douleur et placebo. Approche anthropologique

Résumé

Dans cet article, l’auteur propose une réflexion sur la perception qu’ont les patients des médicaments placebos et de leur prescription par les médecins. Cette réflexion se fonde sur des matériaux recueillis lors d’une recherche anthropologique sur le rapport aux médicaments, aux ordonnances et aux médecins, chez des patients de différentes origines culturelles religieuses. S’appuyant plus particulièrement ici sur les données relatives aux patients d’origines catholique ou protestante, l’auteur met en évidence les diversités des représentations des malades et de leur rapport aux médicaments en général, mais aussi aux médicaments génériques et aux médicaments psychotropes en particulier, de leur rapport à la douleur, au corps, et au mensonge, auxquels sont inextricablement liées les représentations du placebo, et montre qu’elles sont l’objet d’une construction sociale et culturelle. La réflexion s’achève sur une analyse des positions éthiques en matière de prescription de placebo, celle des «principistes» contre celle des «utilitaristes», qui se fondent sur des liens différents avec la question de l’efficacité.

Summary

In this article, the author proposes a reflexion on the perception that the patients have of the placebos and of their prescription by the doctors. This reflexion is based on materials collected during an anthropological research on the relation to drugs, prescriptions and doctors, among patients of various religious cultural origins. Based more particularly here on the data relating to the patients of Catholic or Protestant origins, the author highlights the diversity of the patients’ perceptions and of their relation to drugs in general, but also to generic drugs and psychotropic drugs in particular, their relation to pain, to the body, and to lying, on which the representations of the placebo are inextricably dependent, and shows that they are the object of a social and cultural construction. The reflexion ends with an analysis of the ethical positions as regards prescription of placebo, that of the «principists» against that of «utilitarists», which are based on different links with the issue of efficacy.
