Résumé
Dans le traitement des douleurs cancéreuses rebelles à la thérapie orale, l’administration des opiacés parfois en combinaison avec la clonidine dans l’espace intrarachidien peut garantir une bonne analgésie ou une analgésie modérée jusqu’à la mort pour la plupart des patients. Vingt patients avec des douleurs neurogènes, somatogènes ou mixtes sont présentés. L’instillation continue de morphine à l’aide d’une pompe externe avec la possibilité de donner des bolus supplémentaires est préférée aux injections à travers la porte d’accès deux fois par jour. L’analgésie est meilleure et les complications comme l’obstruction du cathéter et la destruction de la voie d’accès sont rares. La somnolence est l’effet secondaire le plus rencontré. La rétention d’urine n’est vue que dans un cas. L’assistance psychologique est un facteur non négligeable qui peut influencer le résultat.
Summary
The intrathecal administration of morphine sometimes in combination with clonidine is used to treat intractable cancer pain which did not respond any more to oral therapy. Twenty patients with a neurogenic, a somatogenic or a mixed type of pain are presented. The continuous intrathecal instillation of morphine with an external pump is preferred to the intermittent bolus injection two times a day. The analgesic results are better and the complications as obstruction of the catheter and destruction of the reservoir are uncommon in the group treated with an external pump. The psychological assistance might be a factor influencing the results.
