Abstract
Deux épisodes d’altération de la conscience avec nausées et vomissements sont décrits après chirurgie rénale percutanée. Us surviennent respectivement deux heures et une heure après la fin de l’intervention. L’extravasation du liquide d’irrigation à base de glycine a 1,5% dans l’espace rétropéritonéal est le facteur déclenchant. Les effets de l’hyponatrémie à 132 et à 129 mmol · L−1 et l’intoxication par la glycine (3518 et 8589 mmol · L−1) sont discutés. Il convient d’insister sur la nécessité dun diagnostic précoce essentiellement clinique, étayé par une enquête biologique rapide afin d’instituer une thérapeutique symptomatique adaptée.
Abstract
We describe two cases of altered consciousness associated with nausea and vomiting after renal surgery. These events occurred respectively one and two hours after surgery. They are believed to be caused by the transfer of large amounts of irrigating solution containing glycine 1.5%. The effects of hyponatraemia (132 and 129 mmol · L−1) and glycinaemia (3518 and 8589 mmol · L−1)) are discussed. We conclude on the importance of an early clinical diagnosis completed by biological investigation to allow an adequate symptomatic management.
