Résumé

Accompagner les patients douloureux chroniques hospitalisés, au sein d’une équipe pluridisciplinaire, telle est la mission des soignants de notre Centre de Traitement de la Douleur.

Un projet de soins comportant différentes étapes est proposé au patient: préparation au temps d’hospitalisation permettant d’obtenir une adhésion à la démarche, accueil et entretiens assurant une mise en confiance et une connaissance approfondie de la personne et de son parcours douloureux. La détermination d’objectifs adaptés et réalisables précèdera la mise en oeuvre du projet personnel, soutenu par les moyens médicamenteux, et non médicamenteux. La relation établie avec le patient reste l’élément moteur de son accompagnement, pour l’amélioration de sa qualité de vie et l’élaboration d’un projet de réinsertion socio-professionnelle.

Summary

Providing support for patients with chronic pain is the role of the nursing staff within the multidisciplinary team of the Pain Centre.

A care project in several stages is proposed to the patient. a preparatory phase at the time of admission to obtain agreement to the procedures; welcoming sessions to establish a climate of confidence and allow the staff to have a thorough understanding of the person and the history of his pain; determination of suitable realistic objectives; and implementation of the personal project, with the support of drugs and especially non-drug means. The relation established with the patient is the driving force behind progress towards a better quality of life.
