Résumé
Certains syndromes douloureux affectant le bas du dos et la région fessière chez les sujets âgés se caractérisent par leur ténacité et le caractère atypique des symptômes. Aux diagnostics habituellement évoqués, il convient d’ajouter certaines étiologies plus rares tel que le syndrome algique iliocostal. Sur la base de deux histoires cliniques caractéristiques, cet article revoit les mécanismes étiopathogéniques, les manifestations cliniques, et l’approche thérapeutique de ce syndrome.
Summary
Painful disorders affecting the low back and the loin in the elderly are sometimes atypical in their clinical presentation and resistant to treatment. In addition to the common diagnosis which are usually involved, some other more rare etiologies have to be considered, such as the costo-iliac pain syndrome. This article reports two typical examples of this syndrome and discusses the mechanisms of pain production, as well as the clinical features, and the therapeutic approach.
