Summary
A 58 year old male with intermittent claudication underwent a reversed saphenous vein bypass autograft for femoropopiteal occlusion in the right lower limb. Two years and eight months later he underwent a similar operation in the left lower limb.
Two years and five months after the second operation claudication returned in the right leg, and aortography revealed a sharply demarcated intraluminal obstruction in both vein grafts, near, but distinct from, the distal anastomosis. The obstruction in the right graft was caused by a localised valve-like diaphragm which almost completely obstructed the vein. In the left graft there was a similar obstruction which proved to be thickening of a valve cusp and fibrosis in the intima in close proximity to the valve.
