Spontaneous hypoglycaemia and diabetes mellitus

Summary

A CASE is reported of a man who, following partial gastrectomy in 1950, developed severe hypoglycaemic attacks. For this he had a distal pancreatectomy in 1966, when the islets were found to be increased in size with β cell hyperplasia. Within six months the severe hypoglycaemic attacks returned; he was diagnosed as a constituted diabetic in February 1970, and this was confirmed by the finding of excess synalbumin antagonism to insulin. In 1972 he developed overt diabetes and is presently being treated with sulphonylureas.
