Résumé
Les auteurs ont étudié en rétrospectif les résultats obtenus après une prise en charge rééducative de 105 patients lombosciatalgiques chroniques. Cette prise en charge a été multidisciplinaire, c'est-à-dire ne s'est pas limitée à la simple dimension physique de la douleur, mais a été adaptée au cas par cas en fonction de l'ensemble des facteurs biomécaniques et psycho-comportementaux mis en évidence lors de l'évaluation clinique initiale.
Les douleurs évoluaient de manière permanente depuis 16,2±14,5 mois; l'appréciation des résultats a été effectuée au terme du séjour, d'une durée moyenne de 4,6±2,1 semaines, mais aussi à distance avec un recul moyen de 12,7±7,6 mois. La majorité des patients (71%) a été soulagée de manière significative et prolongée. Chez les 77 patients en arrêt de travail (depuis 13,5±11,5 mois), une réintégration du monde professionnel a été possible chez 57 d'entre eux (74%).
L'analyse critique des résultats confirme de plus la valeur prédictive et l'intérêt stratégique d'une évaluation multi-factorielle, au minimum bimodale, somatique et psychologique, idéalement plurimodale, en termes de déficiences, incapacités, handicap et de qualité de vie, avec une observation clinique permettant des mises en situations.
Summary
The authors retrospoctively studied results from the functional restoration program of 105 consecutive patients with chronic lumbosciatic pain. This was a multidisciplinary management, because it did not limit itself to simple pain physical measurement but was adjusted to each case, taking into all the mechanical and psychosocial factors proved out during the first clinical assessment.
Pains had been developing for 16.2±14.5 months. Result assessment was made at the end of the stay, which had lasted on the average 4.6±2.1 weeks, but also later on, after 12.7±7.6 months. Most patients (71%) were relieved in a significant and lasting way. 57 (74%) out of the 77 patients which had been on sick leave (for 13.5±11.5 months) were able to return to the workplace.
Moreover a critical analysis of the results gives a predicting value to a multifactorial at least bimodal, somatic and psychological, ideally plurimodal assessment as far as deficiencies, disabilities, handicap and quality of life are concerned, allowing clinical work observance.
