Le concept anglo-saxon de «tension type headache» est maintenant reconnu internationalement.
Il correspond à une description clinique relativement floue et variable où la classification IHS retient des formes épisodiques et des formes chroniques, ce qui correspond à une expérience clinique. Par contre, la distinction entre les formes avec ou sans pertubation des muscles péricrâniens paraît arbitraire à beaucoup.
Le concept français de céphalées psychogènes ne se superpose pas exactement à celui de céphalées de tension et n'en constitue qu'une partie. Les arguments en faveur d'un continuum ou d'une dualité entre migraine et céphalées de tension sont passés en revue, la dualité paraissant plus vraisemblable, même si certains mécanismes peuvent être communs aux deux entités.
Summary
The anglo-saxon concept of «tension type headache» is now internationally recognised. It fits a quite vague and variable clinical description where the IHS classification has episodic and chronic forms, which squares with a clinical experience. On the other hand, some see as arbitrary the distinction between the forms with or without disturbance of the pericranial muscles.
The French concept of psychogenic headaches does not exactly fit with the one of tension type headaches and is only a part of it. Arguments in favour a continuum or a duality between migraine and tension type headaches are successively considered. Nevertheless, the duality seems to be more probable, even if some mechanisms can be shared by both entities.
