Hémostase par voie endoscopique: méthodes, indications, résultats

Conclusions

En cas d’hémorragie digestive, tout doit être fait dès que possible pour obtenir une hémostase endoscopique immédiate et si possible définitive. Pour cela, le gastroentérologue dispose de nombreuses techniques variées qu’il peut éventuellement associer. Toutes ces techniques ont une grande efficacité hémostatique. Dans les circonstances de l’urgence, le choix de la technique doit avant tout tenir compte de la cause de l’hémorragie, de l’expérience de l’opérateur et des conditions de réalisation de l’acte endoscopique. En cas de récidive hémorragique, il faut savoir renouveler un traitement endoscopique lorsque les chances d’obtenir une hémostase définitive sont élevées mais également discuter d’autres méthodes hémostatiques dans le cas contraire. La CPA est indispensable dans ce contexte car elle cumule de nombreux avantages. La pose de clips, lorsqu’elle est maîtrisée, constitue également une méthode irremplaçable dans certaines indications. Dans l’hémorragie ulcéreuse comme dans les hémorragies de l’hypertension portale, l’association d’un traitement médical spécifique à la prise en charge endoscopique augmente les chances de succès en terme d’hémostase définitive.
