Résumé
L’enfant et le nouveau-né présentent des particularités quant à leur système hémostatique, comparativement à l’adulte. En effet, l’enfant pendant sa première année de vie, et plus particulièrement en cas de prématurité, présente des taux de facteurs pro-coagulants (tous sauf le facteur VIII et le facteur Willebrand) et des taux d’inhibiteurs physiologiques (antithrombine, protéine C, protéine C, protéine S), significativement plus bas que chez l’adulte (1, 2). Les résultats biologiques doivent donc être interprétés en fonction de ces caractéristiques physiologiques (Tableaux I à IV).
La deuxième particularité en pédiatrie concerne la stratégie d’exploration de l’hémostase, conditionnée par les petits volumes échantillon prélevés chez l’enfant.
Nous aborderons l’exploration de la coagulation en contexte pré-opératoire, la finalité de ce bilan pré-opératoire étant de dépister une anomalie potentiellement hémorragipare. L’hémostase primaire et le bilan le thrombophilie seront également abordés dans un deuxième temps.
