Abstract
The disadvantages of the classical approach to the mastoid and middle ear by the postauricular method are outlined. The author considers that an endaural approach combined with bone removal commencing at the bony tympanic annulus is a simpler and much safer technique. This method should be incorporated in the training of all young otolaryngologists. Important points of operative technique are discussed.
