Résumé
La bilharziose à S. intercalatum a pour particularité d’atteindre électivement le rectum.
L’endémie est localisée uniquement en Afrique Centrale.
Le cycle parasitologique, l’infestation et l’invasion sont semblables pour tous les types de bilharzies.
C’est à la phase d’état que la ponte des œufs par tes femelles au niveau des plexus hémorroïdaux va entraîner des granulomes dans la paroi rectale avec des aspects endoscopiques typiques : granulations, polypes bilharziens, ou moins spécifiques : hyperhémie, suffusion hémorragique, ulcérations.
Une étude menée au C.H.U. de Libreville au Gabon sur 405 examens rectosigmoïdoscopiques, permet de retrouver 26,6 % de bilharzioses à S. intercalatum, soit 108 malades. 37 % d’entre eux venaient consulter pour rectorragies. Dans 8 % des cas il existait une rectite grave avec spoliation sanguine.
Le traitement actuel simple et efficace est le Praziquantel en prise unique.
Summary
One of the characteristics of the intestinal schistosomiasis by S. intercalatum is that it strikes the rectum exclusively.
The disease is found only in Central Africa.
The parasitological cycle, the infection and the spread of the disease are the same as for all types of schistosoma.
During the phase when the female lays her eggs in the hemorroidal plexus, granules with typical endoscopic aspects appear on the rectal wall : granulations, schistosomic polyps, or less specially : hyperhemia, hemorrhagic suffusion, ulcérations.
A study carried out at the General Hospital of Libreville, Gabon, based on 405 sigmoidoscopic rectal examinations, resulted in 26.6 % of schistosoma intercalatum infection, i.e., in 108 patients, of which 37 came for consultation due to « rectal haemorrhage ».
In 8 % of these cases there existed serious rectal lesion with blood spoliation.
The current treatment, simple and efficient, is a one-time dose of Praziquantel.
Resumen
La bilarziosis por S. intercalatum tiene como particularidad la de afectar exclusivamente al recto. La endemia se localiza exclusivamente en Africa Central. El ciclo parasitológico, la infestación y la invasión son parecidos a otros tipos de bilarziasis.
En el período de estado se produce la puesta de huevos por parte de las hembras a nivel de los plexos hemorroidarios, lo cual conduce a la formación de granulomas en la pared rectal con aspectos endoscópicos tipicos : granulation, polipos bilarzianos o otros menos especificos : hiperemia, sufusión hemorrágica, uleraciones.
En un estudio realizado en el C.H.U de Libreville, en Gabón, se comprobó que sobre 405 exámenes rectosigmoidoscópicos hubo un 26,6 % de bilarziosis por S. intercalatum, lo que représenta la afectación en 108 pacientes. Un 37 % de los mismos consultaban por rectorragias. En un 8 % de casos hubo rectitis grave con importante pérdida de sangre.
El tratamiento actual más simple, y eficaz consiste en la administración de praciquantel en dosis única.
