Summary
The window trocar, made by Olympus-Winder-IBE following indications, gived by the authors enlarges the diagnostic possibilities of the peritoneal endoscopic examination and presents two main advantages.
First, it allows a thorough exploration of the tissues lesioned by the insertion of the trocar. The various layers of the transparietal channel, can be directly from the skin up to the peritoneum. In this way the risk of bleeding from the abdominal wound is averted.
Secondly, it has been made possible to remove the blind area of the parietal peritoneum near the insertion point of the trocar. Using the conventional instruments, this area, extended from 5 to 10 cm in diameter around the abdominal insertion point, is practically impossible to be inspected. The new trocar consents an easy inspection of the whole parietal peritoneum including the blind area. This is especially relevant in case of portal hypertension and also of localized productive or inflammatory lesions.
The overall experience with this instrument covers the last five years without inconvenience neither complication.
The new trocar is particularly helpful in the hands of poorly trained laparoscopists.
Résumé
Les auteurs décrivent un nouveau trocart, réalisé par Olympus-Winter-IBE selon leurs instructions. Ce trocart présente la caractéristique d’une cannule fendue à son extrémité distale par deux fenêtres latérales opposées.
Les avantages de ce trocart sont:

Une vision directe des tissus pariétaux traversés par le trocart et la possibilité de reconnaître un vaisseau lésé, d’en réaliser la compression temporaire en attendant l’hémostase chirurgicale.

La vision à travers les fenêtres de la cannule permet d’observer le péritoine pariétal au niveau de la zone habituellement aveugle, de 5 à 10 cm de diamètre située autour du point d’insertion du trocart. Cette vision présente un intérêt particulier dans l’hypertension portale et les lésions inflammatoires localisées. Ce modèle de trocart utilisé pendant une période d’environ 5 ans, n’a entraîné aucune complication et sa manipulation s’est avérée même dans les mains de laparoscopistes débutants.
