Laparoscopy in gall bladder carcinoma

La laparoscopie dans le cancer de la vésicule billaire

Summary

Twenty three patients with gall bladder carcinoma were critically evaluated by laparoscopy. The diagnosis was confirmed in each case by either preoperative histology and/or cytology or surgery. On the basis of laparoscopic appearance the criteria for diagnosis of gall bladder carcinoma were classified in terms of direct and indirect evidence. Direct included a mass lesion involving gall bladder or nodules over gall bladder and/or along the adjoining area of the liver. A hard irregular mass in the region of gall bladder covered by omentum along with tortuous blood vessels should be considered as presence of indirect evidence. Direct evidence of carcinoma was present in 15 (65%) and indirect in 8 (35%) patients. In presence of direct evidence of carcinoma cytology only was positive i n80% biopsy alone in 60% and the two techniques in combination yielded tissue diagnosis in 86% of patients. The yield of positive biopsy or cytology was not remarkable in presence of indirect evidence. Laparoscopy also helped in avoiding surgery in 39 % of patients with gall bladder carcinoma due to the presence of multiple metastatic lesion over the liver surface.

Résumé

23 patients atteints de cancer de la vésicule biliaire ont été l’objet d’une évaluation critique du bilan laparoscopique. Le diagnostic a été confirmé dans chaque cas soit par l’histologie et/ou la cytologie pré-opératoires soit par la chirurgie. En termes d’aspect laparoscopique, le diagnostic de cancer de la vésicule biliaire a été classé comme évident ou évocateur avec signes indirects. L’évidence directe repose sur le constat d’une masse tumorale englobant, la vésicule biliaire ou de nodules à la surface de la vésicule et/ou du parenchyme hépatique voisin. L’existence dans la région vésiculaire d’une masse dure, irrégulière, couverte d’adhérences avec péri-vascularisation importante, constitue un indice indirect. Le diagnostic évident de cance a été posé chez 15 malades (65 %) et fut évoqué pour signes indirects chez 8 malades (35%). En cas de diagnostic évident, la cytologie et la biopsie étaient positives isolément, respectivement dans 80% et 60% des cas et les 2 techniques associées ont contribué au diagnostic définitif chez 86 % des patients. Les proportions de biopsies et/ou cytologie positives n’étaient pas significatives en présence de signes diagnostiques indirects. La laparoscopie a également été contributive par prévention de l’intervention chez 39 % des patients atteints de cancer vésiculaire avec métastases hépatiques multiples.
