Antibiotic-induced pseudomembranous colitis morphologic criteria for diagnosis

Résumé

Il est possible que dans un avenir très proche, le diagnostic de colite pseudomembraneuse secondaire aux antibiotiques soit bactériologique.

Cependant, dans l’état actuel des techniques, l’endoscopie, avec biopsie systématique reste le moyen de diagnostic le plus simple et le plus rapide. Elle doit s’accompagner d’une radiographie de l’abdomen à blanc dans le but de préciser l’extension des lésions vers le côlon et de déceler une éventuelle colectasie. Le lavement baryté ou la coloscopie ne sont indiqués que dans les formes où la rectoscopie est négative.

Summary

In the near future, diagnosis of antibiotic-induced pseudo-membranous colitis may well be based on bacteriologic studies.

In the present state of the techniques, however, endoscopy withs ystematic biopsy remains the easiest and fastest diagnostic procedure. It should be complemented with a plain film of the abdomen in order to determine the extension of the lesions towards the colon and to detect possible colectasies. Baryum enema or coloscopy are only indicated when the rectoscopy remains negative.
