Résumé
Les auteurs ont effectué une surveillance clinique et endoscopique chez 46 ulcéreux duodénaux traités par cimetidine. A la fin d’un cycle thérapeutique de 28 jours (cimetidine: 1 g/jour), 37 sur 46 patients (80,4%) ont guéri leur ulcère. Dix-sept d’entre eux ont interrompu la thérapie et après 3 mois ont présenté une rechute clinique dans 76,4% des cas et endoscopique dans 64,7%. Les autres 20 cas ont été traités avec 400 mg de cimetidine par jour pendant 3 mois: durant cette période se sont manifestées 3 (15%) rechutes cliniques et 2 (10%) endoscopiques.
Après 28 jours de thérapie avec antiacides 10 sur 16 ulcéreux duodénaux du groupe de contrôle étaient guéris à l’endoscopie: 90 jours après la suspension de cette thérapie ont été observées 6 (60%) rechutes cliniques et 4 (40%) endoscopiques de l’ulcère.
Les données suggèrent l’utilité dans l’ulcère duodénal, d’une thérapie initiale avec cimetidine à la dose d’un gramme par jour pour 28 jours, suivie d’un traitement prolongé à plus bas dosage (400 mg/jour).
Summary
The Authors performed a clinical and endoscopical follow-up of 46 duodenal ulcer patients treated with cimetidine.
After a short term cimetidine treatment (1 g/day, for 28 days), 37 out of 46 patients (80,4%) showed healed ulcers. Twenty-seven of them were kept without therapy for 3 months, with clinical relapse in the 76,4% of cases and endoscopical relapse in the 64,7%. The other 20 patients underwent long term therapy with 400mg. of cimetidine taken at bedtime, and showed up to 3 months, 3 (15%) clinical and 2 (10%) endoscopical relapses.
Ten out of 16 (62,5%) duodenal ulcer patients of the control group had healed ulcers after short term antacid therapy: 90 days after the complete interruption of this treatment 6 (60%) clinical and 4 (40%) endoscopical relapses were found.
These data indicate the efficacy of the short term treatment of duodenal ulcer with cimetidine at the dose of 1 g/day for 28 days, followed by a long term therapy at a lower dose (400 mg/day).
