Résumé
Un grand nombre de patients avec la maladie de Crohn présentent dans le cours de leur histoire des lésions anales (76%) soit comme une manifestation isolée de la maladie, ou en relation chronologique variable avec des lésions intestinales. L'aspect clinique de ces lésions anales peut être tout à fait trivial (55%) ou, par contre, se caractériser par un polymorphisme typique (45%) c'est-à-dire irritation périanale intense, œdème cutané, ulcères longitudinaux et profonds, fistules et abcès, des ponts cutanés et muqueux, parfois même une sténose anale. A l'opposé d'autres auteurs, nous sommes convaincus, qu'un traitement chirurgical radical c'est-à-dire e. a. le débridement complet associé à un traitement soigneux des lésions cutanées et des dilatations anales postopératoires prolongées, donne d'excellents résultats.
Nous avons obtenu une guérison complète dans 68% des cas. La chirurgie extensive a été couronnée de succès dans 82.3% de ces cas.
Dans quelques cas la colostomie de déviation a pu être refermée et nous avons même pratiqué des plasties spinctériennes. Deux patients ont subi avec succès un traitement pour des fistules au niveau, de la colostomie ou l'ileostomie.
Avec la présentation de nos résultats, nous insistons sur l'importance d'un, traitement chirurgical précoce de telles lésions qu'elles soient simples ou compliquées.
Summary
An important number of patients with Crohn's disease suffer, at some time in the course of their disease, from anal lesions (76%) which can occur as an isolated manifestation of the disease or in some chronological connection with Crohn's lesions at other sites.
The clinical features vary from absolutely trivial, anal pathology (55%) to pathognomonic lesions (45%) i. e.: a typical polymorphism: intense perianal irritation, oedematous skin tags, deep longitudinal ulcers, fistulous tracts and abscesses, skin and mucosal bridges, eventually stenosis of the anal canal.
Unlike some authors, we are convinced that a radical surgical treatment like, thorough debridement, careful treatment of the perianal skin and prolonged postoperative dilatations, provides a good chance for cure of these lesions. We obtained a complete healing in 68% of all our cases. The cure rate for the extensive surgery was 82,3%.
In some cases closure of a diverting colostomy, and even sphincter-repair is possible.
In two patients surgical treatment of colostomy and ileostomy Crohn fistulas was successful.
Presenting our results, we insist on the importance of a surgical treatment at an early stage, of these lesions whether they are simple or complicated
