Diagnostic approaches in the follow-up of ileo-colonic anastomoses in Crohn's disease

Résumé

L'étude présentée a pour but d'évaluer la valeur l'efficacité et la crédibilité des différentes méthodes de diagnostic (données cliniques, tests de laboratoire, examens radiographiques et endoscopiques) dans la surveillance de patients soumis à une résection intestinale pour maladie de Crohn et porteurs d'une anastomose iléo-colique.

L'évaluation rétrospective porte sur 22 cas opérés entre 1 et 7 ans auparavant, pour maladie de Crohn avec atteinte de l'iléon terminal et du côlon droit, traités par hémicolectomie droite et anastomose iléo-transverse. Les résultats démontrent l'intérêt majeur de l'endoscopie dans le diagnostic précoce des récidives anastomotiques, à un stade où l'évaluation clinique, les tests de laboratoire et la radiologie sont peu ou non contributifs.

Summary

The aim of this study is to evaluate the significance, the efficacy and the validity of various diagnostic approaches (including clinical assessments, laboratory investigations, X-rays and endoscopy) in the follow-up of patients who underwent intestinal resection and ileo-colonic anastomosis for Crohn's disease.

A retrospective review is presented of 22 cases operated from 1 to 7 years previously for Crohn's disease of the terminal ileum and right colon who underwent right hemicolectomy with ileo-transverse colon anastomosis. Results reveal the marked significance of endoscopy for early diagnosis of relapse at the anastomosis, not evident by a clinical examination, by laboratory tests or X-rays.
