Pseudo-sarcoma and leiomyosarcoma of the esophagus: role and limits of endoscopic macrobiopsy

Résumé

Les tumeurs malignes polypoïdes de l'œsophage sont rares, et leur diagnostic histologique a été fait le plus souvent après exérèse chirurgicale, les biopsies endoscopiques à la pince étant généralement insuffisantes. Les auteurs rapportent un cas de pseudo-sarcome et un cas de léiomyosarcome dont le diagnostic histologique a été fait par l'étude d'un fragment important obtenu par exérèse à l'anse diathermique. Cette technique de prélèvement a permis en particulier le diagnostic de léiomyosarcome après étude en microscopie électronique. La fonte spectaculaire, bien que transitoire, de ces deux tumeurs après radiothérapie est signalée.

Summary

There are few cases of polypoid malignant tumors of the esophagus, and their histologic diagnosis is usually from surgical specimen, endoscopic biopsies by means of forceps being generally insufficient.

One case of pseudosarcoma and one of leiomyosarcoma are reported by the authors. A fragment large enough for histologic diagnosis was excised endoscopically with a diathermic snare.

This sampling technic as well as electron microscopy, have made possible the endoscopic diagnosis of leiomyosarcoma.

A spectacular though temporary disappearance of these 2 tumors was obtained with radiotherapy.
