La duodénoscopie dans le diagnostic de l'ulcus duodénal

Résumé

Une duodénoscopie hypotonique a été réalisée 58 fois avec succès dans une série de 65 patients suivis pour ulcus duodénal chronique.

Une niche évolutive a été mise en évidence chez 53% des malades examinés. L'impossibilité de franchir le pylore (11% des cas) et l'existence de rétractions cicatricielles du duodénum (38% des cas) limitent les possibilités de la duodénoscopie dans l'examen du bulbe ulcéreux. L'auteur discute les avantages et les inconvénients des différents fibroscopes utilisés.

Summary

Hypotonic duodenoscopy was performed successfully 58 times in 65 patients followed for chronic duodenal ulcer.

An evolutive ulcer was observed in 53% of the patients submitted to duodenoscopy. Inability to push the tip of the instrument through the pylorus impaired duodenal examination in 11% of the cases and duodenal scars have limited possibilities of complete bulboscopy in 38% of the patients. Value and limits of each instrument used in this serie are discused.
