Résumé
L’aganglionnose distale et ultracourte est une forme rare et d’expression tardive de la maladie de Hirshprung. Si l’intervention de Duhamel est encore pour la plupart des auteurs la technique de référence dans cette pathologie, une autre approche thérapeutique a été décrite, privilégiant le traitement de l’achalasie distale et associant la résection colorectale à une sphinctéromyectomie.
Six patients (cinq enfants et un adulte jeune) ont été opérés selon cette stratégie avec un résultat fonctionnel remarquable et une morbidité spécifique nulle. Dans les conditions techniques de notre exercice en Afrique, le prélèvement d’une bandelette musculaire sphinctérienne en pleine zone pathologique présente en outre l’intérêt majeur de permettre un diagnostic positif histologique de la maladie, en l’absence d’autres moyens d’investigations.
Summary
Ultrashort distal aganglionosis is a rare form and late expression of Hirshprung’s disease. Although Duhamel’s procedure is still the reference technique for the majority of authors in this pathology, another therapeutic approach has been described in which the distal achalasia is treated primarily, by associating colorectal resection with a sphincteromyectomy.
Six patients (5 children and a young adult) underwent surgery according to this strategy and the functional results were both remarkable and without the slightest specific morbidity. With the technical conditions available in our practice in Africa, the removal of a strip of sphincteral muscle in the middle of the pathological zone has the major advantage of allowing a positive histological confirmation of the diagnosis of the disease to be made, in the absence of other investigative means.
