Résumé
Une revue de la littérature est faite sur les performances de l’échoendoscopie pour le bilan d’extension local du cancer pancréatique, ces performances sont comparées à celles des autres techniques d’imagerie. L’échoendoscopie s’avère être la méthode la plus précise en ce qui concerne les petites tumeurs, le bilan d’extension des lésions dont la taille est supérieure à 4 cm étant correctement assuré par échographie externe et tomodensitométrie. Cette haute performance, permettant un bilan d’extension fiable, est obtenue au prix d’une expérience en échoendoscopie devant faire face à certaines nécessités techniques. Tous ces problèmes théoriques et pratiques rencontrés en échoendoscopie par le praticien pour le bilan local d’extension des cancers pancréatiques, notamment en fonction de leur localisation, sont abordés dans cet article. L’échoendoscopie devrait donc aider à améliorer la prise en charge des patients atteints de cancer pancréatique: une meilleure sélection des patients devrait être effectuée afin de détecter les tumeurs potentiellement résécables et d’orienter le traitement, soit vers une chirurgie d’exérèse à visée curative, soit d’emblée vers un traitement palliatif associé ou non à des traitements adjuvants.
Summary
A review of literature is made concerning the performances of endoscopic ultrasonography, for the local staging of pancreatic cancer, these performances are compared to those of other imaging procedures. Endoscopic ultrasonography appeared to be the most precise method for the staging of small tumors, staging of tumors more than 4 cm in size being correctly performed by external ultrasonography and Cf scan. This high performance is obtained pending an experience in endoscopic ultrasonography and its specific technical requirements. All the theoretical and practical problems met with endoscopic ultrasonography by the practitioner for local staging of pancreatic cancer, especially in its different localizations, are detailed in the present paper. Finally, endoscopic ultrasonography should help to improve the management of patients with pancreatic cancer: a better selection of patients should be done in order to detect potentially resectable tumors and then to propose either a surgical resection in a curative way, or a first line palliative treatment associated or not with adjuvant therapy.
