Résumé
Les auteurs ont relevé 23 cas de fistules cholédochoduodénales parapapillaires (2 %) dans une série de 1 144 C.P.R.E. au cours de la période 1978-1986.
Dans 22 cas, une cholécystectomie-cholédochotomie avait été réalisée auparavant. Dans chaque cas, les chirurgiens ont utilisé des sondes métalliques afin ďévaluer la perméabilite de la papille en cours ďopération.
Les auteurs décrivent ľorigine de ces fistules traumatiques iatrogènes ; ils recommandent le recours à ďautres méthodes modernes de diagnostic. Le cathétérisme de la papille par sonde est dépassé, source ďerreurs et dangereux.
Summary
The authors have found parapapillary choledochoduodenal fistulas in 23 cases (2 %) while performing 1 144 E.R.C.P.s between 1978-1986. Cholecystectomy, in 22 cases choledochotomy was done previously. In each case the surgeons have used metal probes to check the permeability of the papilla of Vater during the operations. The authors describe these artificial fistulas of iatrogenic origin ; and recommend the use of alternative modern diagnostic methods. They consider probing the papilla of Vater out-of-date, misleading and dangerous.
