Endoscopy in HIV infected children

Résumé

L’endoscopie est considérée comme un examen de choix dans l’évaluation et parfois même le traitement de certaines manifestations gastro-intestinales de l’infection HIV de l’adulte. L’expérience pédiatrique, même si elle reste encore limitée dans ce domaine, semble confirmer le rôle prépondérant de l’endoscopie dans la recherche d’un diagnostic etiologique en présence de symptômes œsophagiens, d’agents infectieux et de signes de dysfonctionnement intestinal en cas de diarrhée chronique, manifestations gastro-intestinales les plus fréquentes de l’infection HIV chez l’enfant. Cet examen n’en demeure pas moins invasif, non dénué de risque, notamment infectieux chez ces enfants immunodéprimés. Il reste réservé aux situations où un diagnostic précis est nécessaire en présence de manifestation gastro-intestinale sévère.

Summary

Endoscopy is considered as the method of choice for the diagnosis, and in certain cases the treatment, of gastrointestinal manifestations of HIV infection in adults. The pediatric experience, although limited, tends to support the crucial role of endoscopy as a diagnostic tool for esophageal symptoms or chronic diarrhea, the most frequent gastrointestinal manifestations in HIV-infected children. In these circumstances, endoscopic examination can demonstrate signs of intestinal dysfunction and lead to the detection of infectious agents. However, the technique is relatively invasive and is not without risks, especially of secondary infections in immunocompromised children. The indications of endoscopic examination therefore remain limited to situations where an etiologic diagnosis is required for severe gastrointestinal manifestations.
