Indications for surgery in childhood Barrett’s esophagus

Résumé

De nombreux arguments plaident pour le développement de l’endobrachyosophage à la suite d’une métaplasie chronique acquise résultant d’un reflux gastro-œsophagien (RGO). L’intervention chirurgicale chez l’enfant porteur d’un endobrachyœsophage est importante en tant que moyen de contrôle des symptômes liés au RGO, pour interrompre la progression de l’épithélium cylindrique, obtenir éventuellement sa régression, et prévenir et traiter l’adénocarcinome développé sur endobrachyœsophage. En raison du fait que chez l’enfant, le risque cumulé de développer un adénocarcinome est supérieur à celui de l’adulte porteur d’un endobrachyœsophage, le traitement agressif du RGO et son contrôle à long terme sont essentiels de même que la surveillance étroite de l’épithélium de Barrett.

Summary

Most evidence favors the development of Barrett’s esophagus as a consequence of chronic mucosal metaplasia resulting from gastroesophageal reflux disease (GERD). Surgical intervention in children with Barrett’s esophagus is important to control the symptoms related to GERD, to halt progression and possibly effect a regression of the Barrett’s epithelium, and to provide a cure for adenocarcinoma of the esophagus arising from Barrett’s epithelium. Because of the larger cumulative years of risk for the development of adenocarcinoma in children as compared to adults with Barrett’s esophagus aggressive and long-term control of GERD in childhood cases is essential along with close follow-up of the Barrett’s epithelium.
